Sessions

Sessions are online only and last 60 minutes unless longer is agreed for a higher fee. Replying to this email means that you agree to me contacting you via email (or phone when absolutely necessary).

Payment

Payment must be made latest at 48 hours before a session by bank transfer. Receipts/invoices are available on request.

Cancellations

I ask for 48 hours notice to cancel your therapy session without incurring a charge.

Non-attendance

If you fail to attend a session without 48 hour notice, the full fee will be required before booking any further sessions.

Reviews

We will review sessions regularly which may be on your demand or as appropriate. You are not tied into any long-term commitment and you may end sessions at any time, however an ending session is recommended. If I consider your needs are beyond the limits of my competence I reserve the right to terminate our contract; this will be discussed in the session and onward recommendations will be provided.

Confidentiality

Your therapy and personal information are kept securely. Information (without disclosing your identity) will be shared with my clinical supervisor who is also accredited with a professional body. Confidentiality may be breached if I consider there is a risk you may harm yourself or others. In such exceptional circumstances, where there is concern for your well being or that of others, it may be necessary to seek help outside the therapeutic relationship. In such an event where I am considering breaching confidentiality, you will normally be consulted first. If this occurs, I aim to discuss these with you and any recommendations will be documented. Also, there are a limited number of circumstances where the law requires confidentiality to be broken, in cases involving terrorist activities, vulnerable adult or child protection issues, drug trafficking or money laundering for example. I’ll decline any request from Social Care, solicitors, or the NHS to write reports or express an opinion on the progress made in the therapy because of my duty of confidentiality to my clients, and because I am not trained in the specialist areas of diagnosis or social work assessment. In addition to this, any notes I take about sessions are extremely brief and very short term, not a verbatim account of who said what and they are used only to support continuity from session to session. However, I can in some circumstances, and on receipt of written consent from you, provide the dates and number of sessions attended. Sometimes court cases or complaints can also mean that confidentiality can be compromised. If I am asked by you, your solicitor, the police, and the courts for access to your records I’ll reserve the right to resist legal requests to produce the records in court as these are not suitable as evidence in legal proceedings and on the ground of confidentiality. The information about confidentiality in no way contravenes your rights under the Data Protection Act 2018 (DPA 2018) and the UK GDPR, collectively known as Data Privacy Legislation, to access personal data that I hold on you.

Recording

Electronic recording of our sessions without my agreement is not allowed.
You taking written or typed notes is encouraged. If you are not comfortable with me taking notes during our session please let me know.

Contraindications

My work with you will not be possible (even dangerous) if there are ongoing legal, criminal or social service proceedings related to your present intimate relationship or ongoing abuse and coercion of any kind. If not sure about this it is very important that you clarify that with me during our first session. If you attempt to work with me regardless of this, I reserve right to immediately stop with our further work and you are not to be entitled to any refund

Also, please note that I do not offer a crisis service. This work is not appropriate for People in Crisis

(experiencing suicidal thoughts and making plans to end life or doing life risking self harming, experiencing severe emotional difficulties (like psychotic episode, manic episode, severe depression, panic attacks …)

Information I collect about you and how I use it

Upon starting therapy, basic personal information will be collected in this agreement for contact and identification reasons. These include your full name, phone, address and your GP details. The assessment and any other sensitive info (History Taking Interview, Scales, Homework Answers and Journals) taken by me from or about you are used solely to support the service offered to you and are stored on fully encrypted on-line space. Basic personal information (see highlighted above) remains on electronic file, off-line and password protected.

Data retention

Your information is kept for the time necessary to provide therapy, however outside of this I will hold your details for a period of time following the end of your therapy to comply with any obligations that are placed upon me by my insurer and my professional organization. At the moment that is 7 years.

Security of your data

Information will be kept securely and confidentially in line with the data retention policy as stated above. Please note I run paperless office. All notes are in digital format only and stored on a my e-reader, which is password protected and always off-line. My short term notes that are for my own reflective use are securely destroyed immediately after the ending of session or therapy, including brief factual notes taken.

Lawful basis for processing your information

The lawful basis for my holding and using your information is in relation to the delivery of a contract to you as a health care professional. As an registered member of COSRT I adhere to their ethical framework and guidelines to ensure that you receive a professional and competent service.
